Here's what I would do:
1) Put a collar on that would prevent him from scratching and opening that wound over and over. I don't like the plastic E-cones, but I do like these:

2) I would put something like Calendula gel on the wound to aid in healing. If it also has aloe and or comfrey in it, all the better.
3) I would clean the ears using Zymox Otic. Google for it and you will find multiple places to buy it. I get the kind WITHOUT hydrocortisone, but that's up to you.
4) If he is eating any grains, I would eliminate them immediately. Don't forget about treats.
5) As for the fleas, since you didn't specify what all you have tried, it's hard to suggest anything. But you might wish to take a look at this article:

My dog has been going through bouts of ear infections. Red, hot, swollen
ear that he has scratched to pieces and now has a deep wound on the outside
of the ear which wont heal from constant scratching. Also, he has
terrible flea allergies, and there are fleas out there when we go walking, and in
the yard. Ive treated the yard and house with all natural remedies, but
still he is suffering. Any ideas on what to treat the deep wound on his ear
with ? It just keeps opening up and bleeding. It is about the size of a
quarter.
